Winston's used to have what I consider the best one in Louisville before they closed for this extensive renovation they have been doing. Hopefully they will reopen and when they do they will still have them. Irish Rover also has a very good one on their bar bites menu. I personally prefer the more traditional ones like these 2 restaurants have instead of the re-creation of one that Game has.

Irish Rover has the classic item. That's the place to go. Mirin has a wild-and-crazy version on its current first anniversary/"grand opening" menu that you might like to catch for the experience, while it lasts. But Irish Rover is the answer to your question.

Gravely Brewing/Commonwealth Cure had one on the Sunday brunch menu today.
