I managed to make it to Puerto Vallarta in Fern Creek and will at least give my opinion. I'm not as good as Robin at this sort of thing.
Cleanliness - It was clean. No, really. I saw no sign of debris on the floor or plates/glasses left on tables. It was a good first impression they wanted people to feel comfortable.
Seating - As soon as I walked in, there was someone by the door; didn't have to wait at all. I was seated promptly in a comfortable booth and asked for a drink order. As I was not sure of what to get, I choose water. While looking at the menu, another employee was passing by and made sure a server had been around to my table.
( Mental note: I don't know if this was a sign for new restaurants, but they seemed to ask me a little TOO much if things were ok. HOWEVER, it did not make me extremely uncomfortable. I knew they wanted to make sure, being newer, a good word had gotten out about their place. )
Food - I'm not sure. What I ate was good. I believed it to be called Puerto Vallarta Special. Two enchiladas with shredded chicken, loads of lettuce, and topped with Guacamole/Sour Cream. It was very good. I ordered the item and it came out not 5 minutes afterward.
The salsa/chips they served before were ok too. I've had better salsas in Louisville, but believed those to have been at store. I knnow one store grow its own peppers and makes their salsa. Most, I think, use premade.
Checkout - The server was prompt in delivering the check; everyone was nice; and I felt it was a good value for the entree.

I'll try more of their dishes and recommend others to give it a shot; especially anyone that lives in the area. As you all well know, this is a food wasteland.
